Applied Clinical Informatics Fellowship
Applicants must have successfully completed an ACGME or AOA accredited program in Family Medicine, Pediatrics or Internal Medicine. We highly recommend submitting an application through the Electronic Residency Application Service (ERAS).

Due to the unique nature of our program, we accept independent applications as specified below. (Please note that all letters of recommendation should be submitted directly by the letter writer, not the applicant.)
Application requirements:
- Complete Texas Standardized Credentialing Application (may omit “Call Coverage and “Practice Location” sections)
- Personal statement
- CV
- 1 program director letter of recommendation
- 2 additional letters of recommendation (If currently in practice, letters should include former program director and 2 colleagues.)
- All USMLE/Comlex scores
- NOTARIZED TRUE COPY of all diplomas and certificates of completion for medical school and any residency and/or fellowship program(s) you have already complete
- Official medical school transcript
- Copy of physician license (all states) or PIT (if previous residency was in Texas)
- Copy of DEA certificate (if fully licensed and have a DEA certificate)
- Copy of DPS certificate (if fully licensed in the State of Texas and have a DPS certificate)
- Copy of ECFMG certificate, if you are an international medical graduate
- Signed attestation about fitness for duty
- Recent photograph
